You may sign up by either calling your Publix Pharmacy Direct Care Call Center at 1-800-PUBLIXRX (1-800-782-5497), or following these three easy steps:

1. Complete the Enrollment Form with the appropriate personal, insurance, and medical information.
2. Sign and return the Assignment of Benefits Form, Billing Authorization Form (if eligible for Medicare), and/or Credit Card Authorization Form (if not eligible for Medicare or if there is a Medicare co-payment).
